Please complete the participation agreement once your student has been accepted to the Odyssey Program.
To register your child for the Odyssey Program for the 2026-2027 school year, please complete the following participation agreement.
Transportation is available for students who reside within the Centennial District boundaries. If transportation is needed, a Transportation Request Form will be sent to you within 2 business days after we receive your Participation Agreement.
You must complete all portions of this form in order to submit the Participation Agreement, which is required for enrollment to the Odyssey Program.
