Immunizations
Minnesota law requires children enrolled in childcare, early childhood education, or school to be immunized against certain diseases, unless the child is medically or non-medically exempt. Please make sure your child is up to date with vaccines by contacting your health care provider and by following up with your students’ school if they have reached out to you regarding your students’ immunization status. If your child is medically or non-medically exempt from immunizations, please see this form for instructions and submit to your child’s school as soon as possible. Please note the form needs to be notarized.
