Kindergarten Information

Minnesota Statutes Section 121A.15 and Minnesota Rules Chapter 4604 require each student enrolled and remaining in an early childhood program, elementary school or secondary school, must submit a record stating they have received immunizations consistent with medically accepted standards.

A record of your child's immunizations dates is required. Clinics can also provide a copy for you upon request. Students having incomplete immunization records will not be allowed to attend school.
