Open Enrollment Statewide Enrollment Form

Application Process

Complete Section 1 on the Statewide Enrollment Options Application. An application must be complete for each student. The parent/guardian address section must contain the parent/guardian address and the address must be in the state of Minnesota.

Sibling preference: Preference is given to applicants who have a sibling who currently attends Centennial Schools. Note in the bottom section of the application the name, grade, and school of all currently attending siblings.

Employee preference: Preference is given to children of employees. Note in the bottom of the section of the application if the applicant is the child of an employee and the name of the employee.

MDE-approved achievement and integration school choice plan: If you are unsure whether the sending district has an achievement and integration plan, see the Minnesota Department of Education website and click on MN Districts with no deadline for open enrollment.

Open Enrollment Forms Language Translation Now Available

The Minnesota Department of Education Open Enrollment website now has language translations of the following open enrollment documents:

  • General Statewide Enrollment Options Application for K-12 and Early Childhood Special Education,  
  • Statewide Enrollment Options Application for State-funded Voluntary Pre-Kindergarten (VPK) or School Readiness Plus (SRP),  
  • Statewide Enrollment Options Instructions, and 
  • School District Non-resident Agreement for Inter-district Enrollment.

The documents are now available in Hmong, Spanish and Somali in addition to English.
