Enrolling Your Student
We look forward to having your student in our classroom!
Required documents needed to register online. See links to the registration website below.
Parent/Guardian Relationship Verification: Court orders, documents related to guardianship or custody limitations, if applicable.
Birth Certificate: A non-certified copy is acceptable. For more information or to order a birth certificate, please click here.
Address Verification: Purchase agreement, lease agreement, or driver’s license with address shown, utility bill or a notarized letter, verifying you are living at the address shown on the registration form.
Previous School Information: School name, address and phone number of schools your student attended.
Emergency Contact: Names and phone numbers.
Pupil Health Immunization Record: Note, children having incomplete immunization records will not be allowed to enter school. Please contact your student’s school nurse for health-related questions. For conscientious objectors, please see information on the Minnesota Department of Health's website on the state's immunization law exemption provision. The form can be found here (see page 2 of the form); sign and have the form notarized.
Early Childhood Screening Verification (Kindergarten enrollments only): Required only if the student did not complete his/her early childhood developmental screening in the Centennial School District.
Education Plans: Submit a copy of 504 Plan, IEP and/or Gifted and Talented, if applicable.
Online Registration Tips:
- Use a computer or a tablet.
- Required fields are marked with a red asterisk, and the district will receive the data exactly as it is entered.
- Please be careful of spelling, capitalization and punctuation.
- Dates should be entered as MM/DD/YYYY and phone numbers as xxx-xxx-xxxx.
- Only type in the house number and apt. number, if applicable.

Need a device? A computer kiosk is available at the District Office, 4707 North Road, Circle Pines, MN 55014.
