Kindergarten Transportation Form

Kindergarten Transportation Form Information

The purpose of this form is to establish bus routes and kindergarten enrollment boundaries for elementary students. This information is also used to assist with planning for student enrollment, student placement, and to ensure a safe and efficient transportation system.

General

School assignments are based on a student’s home address.  An alternate address may be selected as the pick up or drop off location if the alternate address is within the same school attendance area as a child’s home and outside the walking boundary.

Alternate pick up and drop off locations must be consistent 5 days per week (Monday – Friday). Transportation will not be provided to or from different addresses on various days.  An alternate address may be selected for before school, after school, or both.

Non-resident kindergarten students may have daycare within the Centennial School District.  If this is the case, transportation may be available. To qualify, parents/guardians must complete and return this form as soon as possible to be considered.

Please contact Transportation at 612.789.5143 with questions regarding this form or if the information provided changes at anytime.
