DIRECTORY INFORMATION
The following is public information: student’s name; photograph; date and place of birth; major field of study; dates of attendance, grade level; enrollment status (i.e., full-time or part-time); participation in officially recognized activities and sports; weight and height of members of athletic teams; degrees, honors and awards received; and most recent educational agency or institution attended. A student’s and their parent/guardian’s home address, telephone number, email address, or other personal contact information is not designated as directory information. Complaints regarding alleged violations of rights accorded parents or students by the federal law may be submitted in writing to: Student Privacy Policy Compliance Office, U.S. Department of Education, 400 Maryland Ave. SW, Washington, D.C. 20202-8520. SCHOOL RESOURCE OFFICERS
Centennial Schools is proud of our strong partnerships with the cities of Blaine and Lino Lakes, which support our School Resource Officer (SRO) program. SROs play an important role in our schools by fostering positive relationships with students and helping to ensure safe and welcoming learning environments. We also value the continued partnership of the Centennial Lakes Police Department, whose collaboration supports our broader safety efforts across the district. Together, our partnerships with all three police departments are vital to our districtwide safety and security planning and demonstrate our shared commitment to student well-being and school safety in Centennial Schools.
BACKGROUND CHECKS
Centennial School District requires that all employees submit to a criminal background check. In addition, the district checks whether the Minnesota Department of Education has taken any disciplinary action against teachers. Some volunteers as well as extracurricular coaches and advisors who are paid or volunteer are also required to submit to a criminal background check. Refer to Minnesota Statute 123B.03 for more details.
TITLE I INFORMATION
The Every Student Succeeds Act (ESSA) gives parents of a student enrolled in a school receiving Title I federal funds (Blue Heron, Golden Lake and Centennial Elementary) the right to know the professional qualifications of teachers funded by Title I federal funds. The act gives parents the right to ask for the following information about affected teachers: licensing for assignment, provisional license information, degrees and majors, and the qualifications of a paraprofessional if providing federally funded services. SPECIAL EDUCATION/SECTION 504
The Centennial School Board is committed to providing a free and appropriate education to all students with disabilities within our district. We ensure that students who qualify under the Americans with Disabilities Act (ADA), the Individuals with Disabilities Education Act (IDEA) of 1990, or Section 504 of the Rehabilitation Act of 1973 are appropriately identified, evaluated, and provided with appropriate educational services. PROTECTION AND PRIVACY OF PUPIL RECORDS
The Centennial School District adopted Board Policy 515 to comply with state and federal laws regarding education records.
COMPREHENSIVE ACHIEVEMENT AND CIVIC READINESS
The Centennial School District’s Comprehensive Achievement and Civic Readiness plan, in compliance with Minnesota Statute 120B.11, is posted online at isd12.org/academics.
ACCEPTABLE USE OF ELECTRONIC RESOURCES
Centennial provides students and staff with access to the district’s electronic resources including computers, networks, internet, and servers. The purpose of these resources is for classroom activities, communication, and educational research. Users are expected to utilize electronic resources to further their educational and personal goals consistent with the mission and policies of the district. Students are provided internet access that employs technology protection measures that block or filter unsuitable content. Parents may request alternative activities not requiring the use of the internet by contacting the school principal.
Use of the district’s electronic resources is a privilege, not a right. Students and staff are provided with notification and an explanation of acceptable use policies and procedures. See Board Policy 524 for more information.
TECHNOLOGY PROVIDERS AND STUDENT EDUCATIONAL DATA
Centennial Schools uses many different technology service providers with curriculum, testing, and assessment. A list of providers along with data privacy policies is available through the tech support resources linked on the parent hub of the ISD12 website at isd12.org/parents.
STUDENT SURVEYS
Student surveys may be conducted as determined necessary by the school district. Surveys will be conducted anonymously. Surveys containing questions pertaining to the student’s or the student’s parent(s) or guardian(s) personal beliefs or practices in sex, family life, morality and religion will not be administered to any student unless the parent or guardian of the student is notified in writing that the survey is to be administered and the parent or guardian of the student gives written permission for the student to participate or has the opportunity to opt out. MINNESOTA’S SCHOOL IMMUNIZATION LAW
Minnesota Statute 121A.15 requires children enrolled in childcare, early childhood education, or school to be immunized against certain diseases, unless the child is medically or non-medically exempt.
If proper documentation is not provided, students will be unable to attend school after October 30, 2026. What you need to do:
• Contact your health care provider and review your child’s immunization status to make sure they are up to date.
• If you missed a required vaccination, make an appointment with a healthcare provider or local clinic as soon as possible.
• Follow up with your student’s school if they have reached out to you regarding your student’s immunization status.
• Turn in medical (must be signed by a health care provider) and non-medical (must be notarized) exemption forms to your student’s school if they are exempt from immunizations.

VOCATIONAL OPPORTUNITIES ANNUAL NOTIFICATION
The Centennial School District offers a variety of vocational opportunities through the Business
Education, Industrial Technology and Family and Consumer Science Departments. The purpose of this notice is to inform students, parents, employees and the general public that these opportunities are offered regardless of race, color, national origin, sex or disability.
Admission in the specific courses is determined by grade level, and in some cases, completion of prerequisite courses. Courses in family and consumer science, welding and small engine, wood, and work-based learning are offered at Centennial High School. Students can also take courses at Northeast Metro 916 in arts, communications and information systems, agriculture, business management and administration, manufacturing and technology, exploration and enrichment, health sciences, human services, and cosmetology. Students can also enrol in emergency medical responder and nursing assistant courses at the Career and Technology Center in Spring Lake Park. A listing of career and educational technology offerings can be found in the registration catalog of courses posted online at highschool.isd12.org.

INTEGRATED PEST MANAGEMENT PESTICIDE NOTIFICATION
The Centennial School District uses a licensed, professional pest control service for the prevention and control of rodents, insects, and other pests in and around district buildings. Estimated schedules of pest control inspections and possible treatments are available for review at each school office. The program consists of: inspection and monitoring to determine whether pests are present and whether treatment is needed; recommendations for maintenance and sanitation to help eliminate pests without pest control materials; use of non-chemical measures such as traps, caulking and screening; application of Environment Protection Agency registered pest control materials when needed.
AHERA NOTIFICATION
In accordance with the Asbestos Hazard Emergency Response Act (AHERA), the Centennial School District has asbestos management plans for each school and are available for inspection. The following activities are scheduled for the 2026-2027 school year: periodic surveillance and asbestos repairs. Call 763-792-6016 for more information.
DRINKING WATER SAFETY
The Centennial School District follows the Minnesota Department of Health’s recommendation of testing potable water supplies every five years. See isd12.org for more information.

The Centennial School District follows the Minnesota Department of Health’s guidelines and recommendations for radon testing in our schools. See isd12.org for more information.
INDOOR AIR QUALITY ANNUAL NOTIFICATION
The Centennial School District has an Indoor Air Quality (IAQ) Management Plan which outlines specific policies and procedures that will be used in the district to address indoor air quality issues. IEA, the district’s health and safety consultant, completes annual IAQ assessments within the school to ensure proper air quality. See isd12.org for information.
