Medications at school

Centennial Schools medication administration procedures were developed to comply with the Centennial School Board Policy, Minnesota Department of Health Guidelines for Medication Administration in Schools, and MN Statute 121A.22. It is designed to protect students, parents/guardians, and school personnel. This form MUST be completed in order for ANY medication (prescription or over-the-counter) to be administered at school. 

Please note: Prescription medication must come from the pharmacy in a current pharmacy-labeled container. This includes epinephrine auto injectors and inhalers. Pharmacy labels must be attached directly to epinephrine pens and inhalers. Over-the-counter medication must come in the originally labeled and SEALED container with the student's name written on it.

All medications must be picked up at the end of the school year by the parent/guardian unless other arrangements are made in advance with the school nurse.
